*New Patient Deposit Policy*
A $50 deposit is required to hold your initial appointment.

• The deposit will be applied toward your copay, coinsurance, deductible or treatment fee.
• If your insurance covers 100% of the visit, the $50 will be refunded to you. The deposit is FULLY refundable if you cancel at least 24 hours before your appointment.
• If you cancel within 24 hours or do not show, the deposit will be applied to the cancellation fee and is non-refundable.
